TERMS AND CONDITIONS OF USE

Overlake Medical Center and Overlake Medical Clinics are pleased to offer you information via a web-based version of our computer systems called MyChart. We think it is important for you to know how we handle information we communicate via the Internet. This Terms & Conditions statement outlines the practices of Overlake Medical Center and Overlake Medical Clinics and our sensitivity to your right to privacy. We reserve the right to revoke access at any time, for any reason.

Response to Electronic Communication - Overlake Medical Center and Overlake Medical Clinics will make its best effort to provide a timely response to electronic inquiries (emails and other electronic communication). In some cases, the clinic staff needing to respond, to an electronic inquiry or other communication, may not be immediately available; therefore, you should allow at least three (3) business days for a response or staff will respond within three (3) business days. Accordingly, emergency situations requiring immediate attention should not be submitted electronically.

Overlake Medical Center and Overlake Medical Clinics are only able to respond to electronic communications based on the information provided by you, the patient. If there is insufficient information provided, we will be unable to provide accurate and reliable services.

These website link(s) are for your informational purposes only. Overlake Medical Center and Overlake Medical Clinics do not endorse and have not verified the accuracy of the information in/on these websites, and you should not rely on any of the information found on the websites for purposes of treatment or diagnosis.

Email Communications - Overlake Medical Center & Clinics may send communications to the email associated with your MyChart account regarding educational or informational communications, such as but not limited to educational newsletters, construction notices and public health announcements. You may also receive email communication to obtain your authorization to receive marketing information. You may opt-out at any time from these communications by following the process as described in this email. By agreeing to these Terms and Conditions, you acknowledge and agree that the use of unencrypted email carries certain risks. For example, email messages could be received by other people who have access to your account or the device that you use to access email. By agreeing to receive communications via email, you acknowledge that these risks exist and expressly consent to receive unencrypted electronic communications as set forth in these Terms and Conditions.

E-mail Privacy - As a user of MyChart you should be aware that you will be notified via e-mail when there is new medical information to be viewed. This means that any person with access to your e-mail will be able to see this notification. This could include your family members, employer or anyone else who can access your e-mail account. Although no private medical information will be sent, the notification that new medical information is available by accessing MyChart may be information that you would not want others to know. Thus, you should take this into consideration when providing an e-mail address or using MyChart.

Parental Access to the online medial record - Proxy Access for a minor’s MyChart account is permitted to individuals who may demonstrate proof of parental status or legal guardianship of the minor. Proxy access to a MyChart account of a minor under age 13 is automatically terminated upon the minor’s 13th birthday. Proxy Access to minors between the ages of 13-17 is not permitted in order to protect the individual’s healthcare rights and to protect specific sensitive information of these individuals. After a minor’s 13th birthday, the minor will need to complete online medical record enrollment to establish a MyChart account of their own.

If you send us an e-mail communication, it may be shared with the Overlake Medical Center and Overlake Medical Clinics clinical staff whom provide care or assist the physician in providing your medical care. Your confidential medical information on MyChart will be accessible only to appropriate Overlake Medical Center and Overlake Medical Clinics physicians and staff.

Security and Confidentiality - Overlake Medical Center and Overlake Medical Clinics afford the same degree of confidentiality to medical information stored on MyChart as is given to medical information stored by Overlake Medical Center and Overlake Medical Clinics in any other medium. Overlake Medical Center and Overlake Medical Clinics are committed to protecting the confidentiality of your medical information. We limit employee access and their ability to enter or view information based upon their role in your care. Firewalls, passwords, encryption, and audit trails are further used to safeguard your information. Overlake Medical Center and Overlake Medical Clinics shall identify the records released and note the time and date of access each time you access MyChart.

Overlake Medical Center and Overlake Medical Clinics have taken steps to make all information received from our online visitors as secure as possible against unauthorized access and use.

For other than general information viewing, MyChart must be accessed with a Secure Sockets Layer (SSL) compatible browser or terminal (Netscape version 4.79 or greater or Internet Explorer versions 5.0 or greater). Our SSL web server uses authentication and offers the highest level of encryption technology commercially available (128-bit RC4).

You can tell when you are secure by looking at the location (URL) field. If the URL begins with https:// (instead of http://), the document comes from a secure server. This means your data cannot be read or deciphered by unauthorized individuals. You can tell whether you are truly connected to Overlake Medical Center and Overlake Medical Clinics by viewing the digital certificate. This certificate verifies the connection between our server's public key and the server's identification.

User names and passwords provide two layers of authentication and are stored in an encrypted database that is isolated from the Internet. As a MyChart user, your role in maintaining the security of your medical information is to: 1) Change your password on a regular basis, and 2) Keep your login ID and password confidential.

Terms and Conditions Revisions

Overlake Medical Center and Clinics reserves the right to periodically revise the MyChart Terms and Conditions. The revised Terms and Conditions will be posted on the MyChart webpage. Your continued use of MyChart confirms your acceptance of the revised Terms and Conditions.
